Brief PaperLESS intake form

Upon scheduling your appointment, as a NEW PATIENT, please complete our digital intake form, found on this website for your convenience. The information you provide us will include general information about yourself and your condition. You report multiple conditions and if you were involved in a motorvehicle collision, you can supply important details of the nature of the injury(s). 

Consultation

Once the digital intake form has been completed and submitted, you will have a scheduled comprehensive consultation with Dr. Weissman to discuss your health history, problems, concerns, as well as recommended treatment options.

History and Examination

In order to address the root cause of your conditions, Dr. Weissman will ask you various questions related to your health. Next, a number of specialized painless tests will be performed to determine which tissues are affected and to what extent.

X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. We have a modern digital Xray unit on premises, so no additional scheduling or traveling needed for diagnostic imaging. 

Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments,  physical therapies, such as electric muscle stimulation, ultrasound, Shockwave (ESTW), laser, spinal decompression and/or soft tissue therapies.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. A customized treatment plan will also be discussed during this time.
